It has been 12 years since the Hand Clinics issue on The Perionychium. Little has changed in the understanding of the anatomy and physiology, but there has been progress in the treatment of acute injuries with grafts and flaps.
Reconstructive procedures have been described for correction of eponychial absences, pincer nails, and free composite grafts. Microscopic transfer of tissue continues to progress, particularly in Japan.
Treatment of nail deformities secondary to distal interphalangeal ganglions (mucous cysts) has shown that distal interphalangeal joint debridement is curative with excellent correction of the nail deformity. Imaging of tumors of the nailbed with magnetic resonance imaging has enabled definitive diagnosis of glomus and other tumors.
The input of dermatologists such as Baran, Krull, and Haneke has been extremely important to the understanding and care of nail problems. Cutting-edge modalities such as osseointegrated studs and snap on nails are discussed. It has been a constantly advancing decade for the nail and those who care for it.
Shepard has retired, and I will by the time another 12 years rolls around, so someone out there with the interest and foresight must step up and carry the torch. Can they find the cause of the ingrown nail and a better treatment for chronic detachment, warts, and the hypertrophic nail?
